Job Description

JOB SUMMARY

Responsible for maintaining, arranging, and assembling patient’s record and keeping them secure and confidential.

DUTIES AND RESPONSIBILITIES

• Shall arrange and assemble the medical records of discharged patients of the hospital.
• Receives medical records of patients discharged the previous day.
• Retrieves charts for follow-up treatment of patients, for doctors, research, and for hospital needs such as medico-legal for court hearing.
• Encode patient’s detail/information to the system.
• Responsible for the releasing and requested medical records to patients.
• Prepares all needed documents as per patient’s request.
• Shall incorporate loose reports/sheets to respective charts.
• Receive and compile all OPD-OR tech.
• Shall maintain the proper filing of the medical records and periodically check files for misfiled records.
• Shall review medical records to ensure that required reports and signatures have been included (quantitative and qualitative analysis).
• Shall maintain surveillance of incomplete medical records and prepare reports of delinquent doctors.
• Shall process birth and death certificates.
• Shall file records according to the established and approved system.
• Segregate the incomplete from complete charts.
• Attends to the problem regarding the submission of discharged charts from the different nurses’ station and admitting office.
• Shall perform other related functions as may be assigned by immediate superior.

KNOWLEDGE AND SKILLS

Education:
• Must be a graduate of any 4-year course with excellent knowledge in physiology and anatomy.
• Graduate of any medical course is an advantage.
• Must have at least worked in the MRS of a hospital or other related field.
• Must have thorough knowledge of medical terminologies.
• Computer literate in MS Word and MS Excel.

Work Experience:
• Must have a 1 year experience in medical records work.

Skills:
• With good oral communication and interpersonal skills.
• Computer literate (MS Word, MS Excel)
• Ability to work well with others
• Ability to work under time constraint
