USRN Care Coordinator

Responsibilities:

- Perform screening/identification and assessment of patients in need of case management services.
- Expected to integrate key elements and development and coordinate a successful transition to care plan
- Provide the following services: education, care coordination, compliance, transition management, utilization management.

Qualifications:

- Bachelor’s degree – BS Nursing
- USRN required
- Must have experience in Clinical case management work in acute care setting
- Proficient with EPIC tool, Meditech, Cerner
- Must have strong patient satisfaction skills
- Experience with geriatric patients with CHF, COPD, pulmonary cardiac issues is a plus.
- Must be willing to work onsite in Ortigas.
