1. Original & Valid NBI
2. Original & Valid Police or Brgy Clearance
3. Valid Professional License
4. Valid ORCR - 2018 above year model starting January 2025
5. If ORCR is not under the biker's name, either of the ff is needed:
- Authorization Letter (with motorcycle details) and ID of owner
- Notarized Deed of Sale
- Certification of Sale or Repo Sale
- Delivery Receipt / Sales Invoice
6. Medical Certificate (FIT TO DRIVE MOTORCYCLE)
