USRN Care Coordinator

Responsibilities:

> Perform screening/identification and assessment of patients in need of case management services.
> Expected to integrate key elements and development and coordinate a successful transition to care plan
> Provide the following services: education, care coordination, compliance, transition management, utilization management.

Qualifications:

> Bachelor’s degree – BS Nursing
> USRN required
> Must have experience in Clinical case management work in acute care setting
> Proficient with EPIC tool, Meditech, Cerner
> Must have strong patient satisfaction skills
> Experience with geriatric patients with CHF, COPD, pulmonary cardiac issues is a plus.
> Must be willing to work onsite in Ortigas.
